Ce mercredi 14 août, l’Organisation mondiale de la santé (OMS) tiendra une réunion cruciale de son comité d’urgence pour évaluer la nécessité de déclencher le plus haut niveau d’alerte en réponse à l’épidémie de monkeypox, également appelée variole du singe ou mpox. Cette réunion intervient au lendemain de l’annonce de l’agence de santé de l’Union africaine, Africa CDC, qui a déclaré une « urgence de santé publique » face à la flambée de cas sur le continent.
Depuis janvier 2022, 38.465 cas de monkeypox ont été signalés dans 16 pays africains, entraînant 1.456 décès. La situation s’est particulièrement aggravée en 2024, avec une augmentation de 160% des cas par rapport à l’année précédente. Cette montée en flèche des infections a conduit Africa CDC à élever le niveau d’alerte au maximum ce mardi 13 août. Le président de l’Africa CDC, Jean Kaseya, a souligné la gravité de la situation lors d’une conférence de presse : « Le mpox a désormais traversé les frontières, touchant des milliers de personnes à travers notre continent (…) Nous déclarons le mpox comme une urgence de santé publique continentale. »
Cette déclaration devrait permettre de débloquer des fonds pour l’achat de vaccins et coordonner une réponse à l’échelle du continent, alors que l’OMS se prépare à prendre une décision similaire à l’échelle mondiale.
Le comité d’urgence de l’OMS, dirigé par Tedros Adhanom Ghebreyesus, évaluera ce mercredi si la situation actuelle justifie de rétablir l' »urgence de santé publique de portée internationale », le niveau d’alerte le plus élevé que l’OMS puisse décréter. Ce statut avait déjà été appliqué en juillet 2022 face à la propagation mondiale du monkeypox, avant d’être levé en mai 2023 lorsque l’épidémie semblait maîtrisée.
Cependant, une nouvelle menace est apparue : une souche plus mortelle et plus transmissible, baptisée « Clade Ib », a été détectée en République démocratique du Congo en septembre 2023. Cette souche, qui cause des éruptions cutanées généralisées plutôt que localisées, s’est rapidement propagée à travers le continent africain, avec un taux de létalité supérieur à 3%. Les enfants de moins de 15 ans sont particulièrement vulnérables, représentant 60% des cas.
En dehors de l’Afrique, la situation est surveillée de près, notamment en France où le nombre de cas signalés a nettement diminué en 2024 par rapport à 2022. Cependant, Santé publique France a rappelé que le virus continue de circuler de manière silencieuse, avec 107 cas détectés entre janvier et juin 2024.
Alors que l’OMS s’apprête à prendre une décision ce mercredi, le monde attend de voir si l’organisation internationale réactivera l’alerte maximale, deux ans après l’avoir fait pour la première fois, afin de faire face à cette nouvelle menace épidémique.
